Joint stiffness in the morning

What It Feels Like


  • Aching, stiff, or restricted joints upon waking — most commonly hips, knees, hands, or spine

  • Reduced range of motion and discomfort that gradually improves with movement and warmth

  • May feel like joints need to "warm up" before normal activity

  • Stiffness lasting more than 30–60 minutes often indicates an inflammatory component


Why It Happens


Joints are lubricated by synovial fluid, which is redistributed through movement. After overnight rest, reduced circulation and fluid movement can cause stiffness, especially in joints with cartilage wear, inflammation, or reduced muscular support.

Common Causes of Morning Joint Stiffness


🔹 Osteoarthritis

Cartilage breakdown in weight-bearing joints (knee, hip, spine) leads to stiffness that's typically worst in the morning and after rest, but improves with movement.


🔹 Inflammatory Arthritis (RA, AS, etc.)

Conditions like rheumatoid arthritis produce prolonged morning stiffness (often 60+ minutes) due to synovial inflammation that peaks overnight.


🔹 Sedentary or Low-Movement Lifestyle

Joints that don't move regularly lose synovial fluid distribution and can stiffen even without structural pathology.


🔹 Deconditioning or Muscle Weakness

Weak supporting musculature around joints allows more compression and reactive tightness, especially after overnight rest.


🔹 Post-Injury or Post-Surgical Changes

Scar tissue, capsular tightening, or altered joint mechanics following injury can cause localized morning stiffness that doesn't resolve without targeted mobility work.

Did You Know?

The duration of morning stiffness is clinically useful. Stiffness that resolves within 30 minutes typically suggests a mechanical or degenerative cause (like osteoarthritis). Stiffness that persists more than 60 minutes is more consistent with inflammatory arthritis and warrants referral for medical workup alongside PT.
