📝 Forms and Superbills

  • 🔐 Client Portal Login – Securely access your documents, schedule, payments, packages, and card info. Watch the getting started video for a quick overview. Trouble logging in? Check your email or give us a call.

  • 🆓 Request a Superbill – Superbills are itemized receipts you can submit to your insurance for reimbursement. You can access them anytime through the secure Client Portal located in Payments. Need a copy emailed? We’re happy to send one via secure link or, with your permission, through standard email if preferred.
     

  • 📅 Book an Appointment – Appointments are required and easy to book online by choosing your preferred service or provider. Not sure where to start? Meet the Zenith team and explore our services first. Need a hand? We’re here to help—just call us at (541) 250-0195.
     

  • 📝 Intake forms & Clinic Policies – Intake forms are sent via secure email link and can also be accessed through the Client Portal. Please complete them before your visit so we can maximize your one-hour of time with us. All service policies, including our cancellation policy, are outlined in the intake forms. Home Exercise Library

  • Convenient Access – We provide our Home Exercise Programs via PDF files or through the Client Portal typically within 1 to 4 hours after your appointment.
     

  • Check back for more information related to online exercises.

Quick Facts

What should I wear to PT or training? - "workout clothes"

What should I wear to my sessions? We recommend comfortably fitting, athletic wear to allow you to move freely. We have two bathroom and space for personal items up front. Shorts and T-shirts are also available at the clinic for those exceptions that occur.

How to reschedule - Client Portal within 48 hours or by phone

What should I do if I need to reschedule? Yes, you can reschedule your appointment in the client portal at least 48 hours in advance or contact us via phone. Cancellation charges may apply, see our policy on the intake documents accessible via the Client Portal or by request.

Can I use HSA or FSA - Yes, check details.

Do you accept HSA and FSA for session? Yes, however please verify not all services may apply for some card holders. Please check with your provider for clarity. Receipts are made readily available 24 x 7 securely in the client portal or by request.

Do I need a referral - No.

Do I need a referral from a Doctor or my Primary Care Physician before evaluations? No, the State of Oregon allows direct access to Doctor of Physical Therapy. We are highly certified in handling unique symptoms and working with extended health care providers when appropriate.
