Elbow pain after throwing

Also Known As: Little Leaguer's elbow, medial elbow pain, UCL stress, throwing elbow, medial apophysitis


What It Feels Like


  • Pain on the inside of the elbow during or after overhead throwing

  • Often worse during the acceleration or follow-through phase of throwing

  • May feel like a sharp ache, tenderness to the touch, or weakness in the throwing arm

  • In youth athletes, can be associated with decreased velocity or loss of control


Why It Happens


Throwing creates significant stress on the inner elbow with every repetition. In youth athletes whose growth plates are still open, this load can cause apophyseal irritation. In older athletes, it more often involves the UCL, flexor-pronator tendons, or ulnar nerve.

Common Causes of Elbow Pain After Throwing


🔹 Medial Apophysitis (Little Leaguer's Elbow)

In skeletally immature throwers, repetitive valgus stress during throwing can irritate the medial epicondyle growth plate.


🔹 UCL Stress or Sprain

The ulnar collateral ligament stabilizes the inside of the elbow during the acceleration phase of throwing. Overuse or high pitch counts can strain or partially tear this structure.


🔹 Flexor-Pronator Muscle Overload

The muscles on the inner forearm are under high eccentric load during deceleration, and fatigue or volume spikes can cause tendon irritation at the elbow.


🔹 Throwing Mechanics Breakdown

Poor hip-trunk rotation, early trunk opening, or dropping the elbow below shoulder height increases valgus stress on the medial elbow with every throw.


🔹 Pitch Count and Fatigue

Throwing through fatigue — especially at high speeds — dramatically increases elbow injury risk in youth athletes.
