PTTD causes inner ankle and arch pain that can progress if it is ignored, especially in runners and active adults. Physical therapy focuses on progressive tendon and foot strengthening plus load management to improve arch support. The goal is pain-free walking and running without ongoing tendon flare-ups.

Common Symptoms
Inner ankle pain; arch pain or fatigue; pain with standing on tiptoes; swelling along the inside of the ankle; worsening pain with long walks or runs.
Common Causes
Rapid increase in walking or running volume; weak calf and foot strength; flat foot mechanics; prolonged standing; footwear changes; prior ankle injury.
How We Treat Posterior Tibial Tendon Dysfunction
We use progressive calf and posterior tib strength work, intrinsic foot strengthening, and gait and footwear strategies to reduce overload. We build walking and running tolerance with symptom-guided progressions.
